29 CFR 1926.252(c): All scrap lumber, waste material, and rubbish was not removed from the immediate work area as the work progressed.  a) Jobsite: First Floor, area in front of building - The employer did not remove all scrap lumber, waste material, and rubbish from the immediate area as the work progressed thus exposing their employees conducting demolition activities to slip, trip, and fall hazards.  Observed on or about 7/8/15.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge 6 feet or more above a lower level was not protected from falling by the use of a guardrail system, safety net system, or personal fall arrest system.  a) Jobsite, 2nd floor to 1st floor, dumpster, and basement - Employees who performed demolition operations from the 2nd floor, approximately 10 - 18 feet above ground level, were exposed to fall hazards without the use of a fall protection system.  Observed on or about 7/8/15.

29 CFR 1926.850(a): Prior to permitting employees to start demolition operations, an engineering survey was not made by a competent person of the structure to determine the condition of the framing, floors, and walls, and possibility of unplanned collapse of any portion of the structure, including adjacent structures where employees may be exposed. The employer did not have in writing evidence that such a survey was performed.  a) Jobsite: An engineering survey was not completed by a competent person prior to permitting employees to conduct demolition operations on a 2 story brick faced structure.  Observed on or about 7/8/15.
