29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:    (a) Jobsite, Oak Park Shopping Center, White Oak, PA:  On or about 07/20/15 a ladder or other safe means of access was not provided to access the scaffold platform.  The employees access the platform and roof area by climbing the ladders used to support the scaffold platform and had to climb over the platform to access the rungs exposing them to fall hazards between 8-12 feet.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:        (a) Jobsite, Oak Park Shopping Center, White Oak, PA:  On or about 07/20/15 employees working on the ladder jack scaffold were not utilizing personal fall arrest systems exposing them to fall hazards of approximately 14 feet to the ground below.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    (a) Jobsite, Oak Park Shopping Center, White Oak, PA:  On or about 07/20/15 employees engaged in roofing activity on a low-sloped roof were not protected from falling approximately 14 feet to the ground below.  There were no guardrail systems, safety nets, personal fall arrest systems, or a monitoring system in place to protect the employees from fall hazards.
