29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. a) As occurring on 07/09/2015, an employee of Ryder Integrated Logistics, Inc. sustained an injury to his hand on while unloading material from a Ryder truck at a CVS store. The employee was admitted as an in-patient, hospitalized overnight, and treated for his injury. The company learned about the hospitalization on 07/12/2015 and failed to report the hospitalization to OSHA until 07/16/2015.
