29 CFR 1904.29(b)(3): Each recordable injury or illness was not recorded on the OSHA 300 Log and 301 Incident Report within seven calendar days of receiving information that a recordable injury or illness has occurred: a) AK Steel Corp. in Mansfield, Ohio; On or about June 11, 2015 the employer failed to record a recordable injury on the OSHA 300 and 301 Incident Report within seven days of receiving information that a recordable injury had occurred. An injury occurred on February 28, 2015 which resulted in work restrictions on June 11, 2015. AK Steel Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1904.29(b)(3), which was contained in OSHA Inspection Number 312635816, citation number 2, item number 1 and was affirmed as a final order on August 6, 2011, with respect to a workplace located at One Armco Drive in Butler, PA 16003.
