29 CFR 1910.178(n)(4): Industrial truck driver(s) were not required to slow down and sound the horn at cross aisles and wherever vision was obstructed: On or about June 29, 2015 at the Hampshire Pet Products facility in Joplin, Missouri, Employees are exposed to the hazard of being struck-by powered industrial trucks that are not being operated safely as required by standard. On June 29, 2015 an employee was injured when the employee was struck by a forklift which resulted in the amputation of the employees leg. Abatement certification for this item is not required.
