29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  (a) This violation occurred on or about June 25, 2015 located at the Southern Endocrinology Office, in the attic, where an employee who working was exposed to an electrical hazard.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  (a) The employer did not report to OSHA within twenty-four hours an employee's in-patient hospitalization as a result of a work related incident that occurred on or about June 25, 2015.
