29 CFR 1910.23(d)(1): Every flight of stairs having four or more risers was not equipped with standard stair railings or standard handrails as specified in paragraphs (d)(1)(i) through (v) of this section.  a) Royal Box Group, LLC - The employer did not ensure that stairs with seven risers leading into the building were equipped with a standard stair railing, including an intermediate rail on each open side.  The east stair railing was not equipped with an intermediate rail.  b) Royal Box Group, LLC - The employer did not ensure that stairs with seven risers leading into the building were equipped with a standard stair railing, including an intermediate rail on each open side.  The west stair railing was not equipped with an intermediate rail.

Rise height and tread width were not uniform throughout the flight of stairs.    a) Royal Box Group, LLC - The employer did not ensure that the wooden stairs with seven treads were slip resistant and the nosings were of nonslip finish.  The employer did not ensure that the rise height and tread width were uniform throughout the flight of stairs.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and cables were used as a substitute for the fixed wiring of a structure:    a)     Assembly Area - A power strip was used to provide power to three battery charging units.  The power strip was connected to an extension cord suspended across the ceiling on a pole and then plugged into an outlet on a column.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  a) Royal Box Group, LLC - The employer did not develop, implement and maintain at the workplace a written hazard communication program in accordance with 29 CFR 1910.1200 that described or included at least the following:  1) Requirement for labeling of containers of hazardous chemicals; 2) Safety data sheet(s) availability; 3) Training of employees; 4) A complete list of hazardous chemicals known to be in the workplace; 5) Methods to inform employees of the hazards of non-routine tasks; and, 6) Methods to inform other employer(s) of safety data sheets availability; the labeling system and any precautionary measures to protect employees.  Employees were exposed to hazardous chemicals including, but not limited to, TECH SPRAY Heavy Duty Adhesive Spray (contains hexane, L.P.G., acetone, petroleum distillates, N-Butane).

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a) Royal Box Group, LLC - did not provide information and training to employees exposed to hazardous chemicals including, but not limited to, TECH SPRAY Heavy Duty Adhesive Spray (contains hexane, L.P.G., acetone, petroleum distillates, N-Butane).  b) Royal Box Group, LLC - did not provide training to employees by December 1, 2013 that included an explanation of the revised safety data sheet (SDS) format and content, as well as the new labeling system elements, as required by the revised hazard communication standard.
