OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:    (a)  At the worksite:  On July 22, 2015 and at times prior to; employees were exposed to struck-by hazards from lifted construction materials in that the below-the-hook lifting device (spreader Bar) that was used to lift steel sections and metal roofing materials was not legibly marked.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems:    (a)  At the worksite at 3690 Carlyle Close, Mobile, AL: On July 22, 2015 and at times prior to; employees were exposed to falls of at least 18 feet, 3 inches while affixing metal sheets on a 3:12 pitch roof with open sides and edges in that employees were not protected from falls by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, fall restraint systems or fall protection plan.

29 CFR 1926.1413(a)(2)(i)(D): A competent person must inspect end connections of the wire rope and ensure those connections are properly applied:  (a)  At the worksite:  On July 22, 2015 and at times prior to; employees were exposed to struck-by a crane load in that a competent person did not properly inspect the terminal end of the truck crane's wire rope in that the dead end of the wire rope passing through the Crosby wedge socket, was secured to the live end of the wire rope with a saddle clamp, creating a pinch of the live rope and an impending failure.
