29 CFR 1926.501(b)(5): Each employee on the face of formwork or reinforcing steel was not protected from falling 6 feet (1.8 m) or more to lower levels by personal fall arrest systems, safety net systems, or positioning device systems: On or about July 22, 2015, during the construction of a road bridge at a job-site located at Riverside Drive, I-90 Bridge, Lakewood, Ohio, the employer did not ensure employees were protected from fall varying in heights of approximately 6 to 19 feet by utilizing a fall protection system/device during the installation and repair of formwork.
