OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees.  a) In the manufacturing area, on or about July 22, 2015 or prior thereto:  Employees were operating a Kalamazoo Startrite 24-T-10 Vertical Band Saw to cut aluminum pieces of up to 1 inch in width.  During this operation, 10 inches of the saw blade were unguarded.   Employees were exposed to the hazard of contacting the portion of the moving saw blade not directly involved with the point of operation.   Abatement certification is required for this item.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written respiratory protection program to ensure that any employee using a respirator voluntarily is medically able to use that respirator, and that the respirator is cleaned, stored, and maintained so that its use does not present a health hazard to the user. Exception: Employers are not required to include in a written respiratory protection program those employees whose only use of respirators involves the voluntary use of filtering facepieces (dust masks).  a) Manufacturing Area, on or about July 22, 2015 or prior thereto:  The employer provided an AO Safety half-face elastomeric organic vapor cartridge respirator to employees and had not established a respiratory protection program.   The respirator was worn during the machining of plastic to filter nuisance odors.  The respirator was not stored in a sanitary manner.  Abatement certification is required for this item.

29 CFR 1910.147(c)(1):  The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  a) Manufacturing Area, on or about July 22, 2015 or prior thereto:  The employer had not established energy control procedures for the 230 Volt Cosen horizontal band saw, Model AH-260C-02, Serial #C920505.   Blade changing is performed on the saw on a quarterly basis.  This item was corrected during the inspection.
