29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge: On July 24, 2015 and times prior to, the Sonitron Alarm System to the Cook Building was faulty; it would inadvertently go off and lock all the exit doors to the building trapping employees inside. This condition exposed employees to fire hazards.
