Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from the potential mixing of incompatible chemicals in the workplace: a) Warehouse, Chemical Storage Cage: On or about July 22, 2015, employee(s) were exposed to chemical hazards from potential mixing of incompatible chemicals, including quaternary ammonium chlorides, such as Vigil-Quat, and oxidizers such as Super Power Foam CL (Potassium Hydroxide, Sodium Hypochlorite, Sodium Hydroxide) and Foam Plus (Sodium Hypochlorite, Potassium Hydroxide). b) Warehouse, Chlorine Dioxide Generation and Storage Room: On or about July 22, 2015, employee(s) were exposed to the potential mixing of Chlorine Dioxide solution, an oxidizer, with Hydrochloric Acid (10-17%), an acid and reducing agent, due to a damaged and leaking 450 gallon chlorine dioxide storage tank.
