29 CFR 1926.251(a)(1): Rigging equipment for material handling was not inspected prior to use on each shift and as necessary during its use to ensure that it was safe: A.) Membrane Inc. dba Membrane Roofers - On or about 7/22/15, and at times prior thereto, the wire rope sling used to lift the hopper was not inspected prior to use. The wire rope sling broke at the eyelets and caused the hopper to fall to the ground. The wire rope sling contained deficiencies that included, but were not limited to, corrosion at the eyelets.

29 CFR 1926.251(a)(6): Each day before being used, and during use, where service conditions warrant, the sling and all fastenings and attachments were not inspected for damage or defects by a competent person, and damaged or defective slings were not immediately removed from service: A.) Membrane Inc. dba Membrane Roofers - On or about 7/22/15, and at times prior thereto, the wire rope sling used to lift the hopper was not inspected prior to use. The wire rope sling broke at the eyelets and caused the hopper to fall to the ground. The wire rope sling contained deficiencies that included but were not limited to, corrosion at the eyelets.
