29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:     a)  On or about 07-22-15, an employee was walking on a stack of roof sheathing supported by the forks of a rough terrain forklift at approximately 12 ft. above surface level.  The Genie GTH 844 Telehandler forklift is not designed for supporting personnel with the forks.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a)  On or about 07-22-15, an employee was walking on a platform supported by a rough terrain forklift at approximately 12 ft. above surface level without utilizing fall protection equipment.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a)  On or about 07-22-15, employees were working on an 8:12 pitched roof at approximately twelve (12) feet above surface level without utilizing fall protection equipment.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.    a)  On or about 07-22-15, employer failed to provided written employee fall protection safety training certification for employees working at heights greater than six (6) feet.  Two employees were working on a roof at approximately 12 ft. above surface level.
