29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed as required by 29 CFR 1960.8(b):  a- US Department of Veterans Affairs, Lyons, NJ - A review of the lockout/tagout procedures for employees performing maintenance/servicing on machines such as but not limited to boilers & compressors was not performed in over a year.    Condition noted on or about 7/22/15.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment as required by 29 CFR 1960.8(b):  a- US Department of Veterans Affairs, Building #15, Carpentry Shop - Electric panel box was blocked by items such as but not limited to a table and pieces of wood.    Condition noted on or about 7/22/15.

29 CFR 1904.7(b)(5): When an injury or illnesses which did not involve restricted work, job transfer or days away from work, the injury was not checked on the OSHA 300, Column "J", as an other recordable injury or illness as required by 29 CFR 1960.8(b):    a- US Department of Veterans Affairs, Lyons, NJ - Column "J" on the OSHA 300 for calendar years 2013, 2014 and 2015 was not checked for the other recordable injuries that did not involve restricted work/job transfer and/or days away from work, which affected the accuracy of the OSHA 300A Summary.        Condition noted on or about 8/6/15.
