29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) In the employers' repair shop in Lebanon, Missouri, an energy control program was not established to protect employees working on equipment which contained stored energy within equipment hydraulic systems. On or about July 15, 2015, an employee working on a John Deere model 825 skid steer loader was injured when a raised bucket on the skid steer loader was not blocked to prevent release of stored energy. The bucket and support arms lowered and caught the employee between the bucket and the frame of the skid steer loader resulting in hospitalization of the employee. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) In the repair shop, procedures had not been developed for employees who were working on equipment having hydraulic systems to protect them against stored energy. An employee was working on a John Deere skid steer, model 825, serial number 010643 GG. An employee was injured when a hydraulic system on the skid steer loader containing stored energy allowed the raised bucket to fall. This resulted in crushing injuries to the employee. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19
