29 CFR 1910.132(d)(1): The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE).   a)  Deacon Industrial Supply Co. - On or about July 22, 2015, employees are exposed to burn hazards from flying sparks and ultraviolet radiation while burning and welding on galvanized pipe, and the employer did not perform a required workplace hazard assessment through a written certification which identified the workplace evaluated, the person certifying the evaluation, the date of the hazard assessment, and identification of the document as a certification of hazard assessment.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

Abatement certification required within 10 days after abatement date. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, rotating parts, flying chips and sparks:    a)  Deacon Industrial Supply Co. - Weld Station #1 - On or about July 22, 2015, employees were exposed to burns and struck-by hazards from an automated, rotating, Plasma/MIG gun which produces flying molten sparks.     b)  Deacon Industrial Supply Co. - Weld Station #2 -  On or about August 26, 2015, employees were exposed to burns and struck-by hazards from an automated, rotating, Plasma/MIG gun which produces flying molten sparks.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.    Abatement Note:  One feasible means of engineering controls for guarding the point of operation on the automated plasma/MIG gun area would be to install a light beam curtain across the front area to prevent hands from being able to put in point of operation.  If hands are present in this area, machine will not activate or shut down.  Additional barrier protection guarding from UV light and sparks could be added by fabricating welding screens to the sides of the machine, since operator steps to side when machine is in operation.
