29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe boards:    a) Thorworks Industries Inc., 2520 Campbell St. Sandusky, OH: On or about July 23, 2015, the employer failed to guard the edges of a raised work mezzanine in the paint coloring department. Employees working on top of the mezzanine were exposed to a fall hazard greater than eight feet.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a)Thorworks Industries Inc., 2520 Campbell St. Sandusky, OH: On or about July 23, 2015, the employer did not ensure that employees were guarded from the rotating shaft of the mixers located in the paint coloring department. Employee are exposed within 12 inches of a rotating shaft while adding material to the paint mixtures during production.
