29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: (a)Sanchez Construction at 3402 Harvard Place, Broomfield, CO 80023: On and before July 18th, 2015, the employer allowed employees to use an orange extension cord and a red splitter that were missing ground plugs, to power portable 120 volt tools that were not double insulated, such as the Skilsaw serial number #402867734. The conditions exposed employees to electrical shock hazards.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): (a)Sanchez Construction at 3402 Harvard Place, Broomfield, CO 80023: On and before July 18, 2015, the employer did not ensure that employees were using fall protection when framing this two-story single family home with a 6 in 12 pitch roof in the following instances: (1)Two employees were working on the roof without fall protection; 2)Employees were working on the main floor near the stairway floor hole to the basement, measuring 62" x 87", without guardrails and fall protection on two sides of the hole; and 3)Employees were working on the second floor near two window wall openings, with the window sills about 2' above the floor, without a top guardrail. The conditions exposed employees to fall hazards of up to approximately 20 feet.
