29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).  (a) Facility, 296 Madison Street, Wilkes-Barre, PA: Employees operated a Komatsu Forklift Model G25T-16 (Serial No. 216183A) weekly and had not received classroom training, practical training or an evaluation of their ability to safely operate the equipment. Employees were exposed to struck-by hazards associated with forklift operations, on or about July 23, 2015.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
