29 CFR 1910.184(e)(2)(i): Hooks, rings, oblong links, pear shaped links, welded or mechanical coupling links or other attachments did not have a rated capacity at least equal to that of the alloy steel chain with which they were used or the sling was used in excess of the rated capacity of the weakest component: At the time of inspection, the employer failed to ensure that the lifting device, that was made in house, had a rated capacity at least equal to that of the alloy steel chain with which was used to lift the cover plates.

29 CFR 1910.184(f)(4)(ii): All welded end attachments shall not be used unless proof tested by the manufacturer or equivalent entity at twice their rated capacity prior to initial use. The employer shall retain a certificate of the proof test, and make it available for examination: At the time of inspection, the employer failed to ensure that the lifting device, that was made in house, had been proof tested at twice it's capacity prior to use.
