29 CFR 1910.134(a)(2): A respirator was not provided by the employer to each employee when such equipment was necessary to protect the health of the employee: On or about July 19, 2015 the employer did not ensure that all employees working near or around rail cars that had the potential to contain hydrogen sulfide above 20 ppm were adequately protected against possible exposure by supplying airline type respirators and escape type respirators.
