Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation hazards:     a) Mexico Plastic Company DBA: Continental Products @ 2000 West Blvd., Mexico, MO 65265: On and before July 16, 2015 the employer did not ensure that employees were protected against amputation hazards related to the withdraw of the Nim-Core Air Shaft from the steel core on the extrusion plastic processing line. An employee, a trainer, withdrew the Nim-Core Air Shaft from the steel core while another employee, a trainee, who was positioned at the opposite end of the Nim-Core Air Shaft, attempted to deflate the air bladder via the air release valve. The trainees left middle finger was caught in the pinch point created between the Nim-Core Air Shaft and the steel core. The stated condition resulted in an amputation of the tip of the trainees left middle finger.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:     a) Mexico Plastic Company DBA: Continental Products @ 2000 West Blvd., Mexico, MO 65265: An employee suffered an amputation of the tip of his left middle finger on July 13, 2015. The employer did not provide necessary notification until July 16, 2015.
