29 CFR 1926.106(c): Ring buoys with at least 90 feet of line shall be provided and readily available for emergency rescue operations. Distance between ring buoys shall not exceed 200 feet.    (a) Location - 10 Blaney Street, Salem, MA:  On or about 7/22/2015, employees working on a concrete walkway adjacent to the harbor were not provided with ring buoys with associated life line in the event of an employee had fallen into the water.

(a) Location - 10 Blaney Street, Salem, MA:  On or about 7/22/2015, employees working on a concrete walkway adjacent to the harbor were not provided with a lifesaving skiff in the event an employee had fallen into the water.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    (a) Location - 10 Blaney Street, Salem, MA:  On or about 7/22/2015, employees working on a concrete walkway that was greater than 6 feet above a lower level were not protected from falling.
