(a) On or about July 24, 2015 the employee restroom was not maintained in a clean and sanitary condition. S &H WAS PREVIOUSLY CITED FOR VIOLATION OF THE OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1910.22(A)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1046217, CITATION NUMBER 1, ITEM NUMBER 2 AND WAS AFFIRMED AS A FINAL ORDER ON 6/24/15, WITH RESPECT TO A WORKPLACE LOCATED AT 2110 HWY. 80 EAST, PEARL, MS 39208.
