29 CFR 1926.850(a): An engineering survey was not performed by a competent person to determine the conditions of the framing, floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations: On or about July 20, 2015, the engineering survey for the demolition of the NorthCoast Behavioral Healthcare Building was inadequate in that it did not account for the building wing construction techniques. The employer strayed from the demolition plan and did not complete a new engineering survey to determine the condition of floors and the possibility of unplanned collapse.

29 CFR 1926.853: The employer did not ensure floors weakened or otherwise made unsafe by demolition operations shall be shored to carry safely the intended imposed load from demolition operations. On or about July 20, 2015, the employer strayed from the demolition plan and did not ensure floors and working surfaces were not made unsafe by demolition operations. The employer placed a Takeuchi TB235 excavator on a floor to perform demolition operations and the demolition plan did not address this change. The floor partially collapsed and exposed employees to struck by/crushing hazards.
