29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was each employee provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    a) On or about July 24, 2015, and at times prior, two workers without fall protection were exposed to fall hazards while performing framing work more than 10 feet above the next lower level.    b) On or about July 24, 2015, and at times prior, four workers without fall protection were exposed to falling more than 6 feet to ground level from an unguarded edge of the main floor deck.

29 CFR 1926.1051(a): A stairway or ladder was not provided at all personnel points of access where there was a break in elevation of 19 inches or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:    a) On or about July 24, 2015, and at times prior, four workers entered the project main where a step up to the main floor from ground level was measured at 27.5 inches.    b) On or about July 24, 2015, and at times prior, two workers were exposed to falling when they climbed 53.25 inches of framed walls between interior top rail and the top of a ladder.
