29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and wider than 50 feet were not protected from falling by the use of a guardrails system, safety net system or personal fall arrest system.  On the roof located approximately at 2511 N 72nd Street, Omaha, NE: The employer is failing to ensure that employees are protected from unprotected sides or edges 6 feet or more above lower levels. On or about July 22, 2015, employees of Rafael Corrales dba Corrales Roofing were performing rubber membrane roofing installation, 13.5 feet above lower levels; on a low-sloped roof without fall protection such as but not limited to guardrail systems, safety net systems, or combination of warning line systems and other approved method as required by the standard.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    Jobsite located at 2511 N 72nd Street, Omaha, NE:  The employer is failing to provide a training program to ensure that employees are able to recognize and minimize hazards when exposed to falls greater than 6 feet. On or about July 22, 2015, employees were observed exposed to fall heights of approximately 13.5 feet without protective measures such as but not limited to guardrails, safety nets and or personal fall protection systems

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On the jobsite located at 2511 N. 72nd Street, Omaha, NE: The employer is failing to ensure that a Werner aluminum extension ladder used by employees extended at least 3 feet past the roof level being accessed and/or is failing to ensure the ladder was secured from accidental displacement.

29 CFR 1926.1060(a):  The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these:    On the jobsite located at 2511 N 72nd Street, Omaha, NE:    The employer is failing to provide employees with training relating to the safe usage of ladders, the hazards associated with unsafe usage and the procedures to follow to minimize the hazards. On July 22, 2015 employees were observed using a 16 foot Werner aluminum extension ladder to access a roof to perform work. The ladder was not secured to prevent displacement and the ladder did not extend at least 3 feet past the roof being accessed
