29 CFR 1926.451(d)(16): In addition to the normal operating brake, suspension scaffold power-operated hoists and manually operated hoists shall have a braking device or locking pawl which engages automatically when a hoist makes either of the following uncontrolled movements: an instantaneous change in momentum or an accelerated overspeed: a) On or about July 21, 2015 - at a jobsite located at 1100 W. Cermak Road, Chicago, IL. - employees were performing tuckpointing operations from a two point suspension scaffold that was not equipped with a braking device or locking pawl that would engage automatically following an uncontrolled movement, thereby exposing the employees to fall hazards of approximately ten (10) feet.

29 CFR 1926.451(d)(17): Manually operated hoists shall require a positive crank force to descend: a) On or about July 21, 2015 - at a jobsite located at 1100 W. Cermak Road, Chicago, IL. - employees were performing tuckpointing operations from a two point suspension scaffold equipped with a manual operating hoist. The hoisting system did not require positive crank force to descend, thereby exposing the employees to fall hazards of approximately ten (10) feet.
