29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    (a)  In the paint area, on or about July 24, 2015 and at times prior to, the employer did not require the use of suitable eye protection for an employee spraying paints and primers inside the spray booth, resulting in a eye injury hazard.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:    (a) In the welding area, on or about July 24, 2015 and at times prior to, an employee voluntarily utilized an Elipsemask respirator with HEPA filters during welding activities without the benefit of a medical evaluation, resulting in a respiratory hazard.
