29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) In the production bay, located at 6182 Idlewild Street, Fort Myers, FL 33966, employees were operating the miter saw, Hitachi C15FB, SN 250037 and SN N40056, with the guard being blocked from covering the entire saw's blade by an aluminum 4 inch wall exposing the employees to operating saw blade, resulting in an amputation on 07/10/2015. Observed on or about 07/15/2015.
