29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):                a) Employees were exposed to the hazard of falling where fall protection was not provided at window/wall openings on the second story of the building.     b) Employees were exposed to the hazard of falling where fall protection was not provided along open sided floors on the second story of the building.     No abatement certification or documentation needed for the above items.     c) Employees were exposed to the hazard of falling while performing carpentry work from the top plate and other surfaces on the second story and roof structure without the use of fall protection.     In accordance with 29 CFR 1903.19(d), abatement certification is required for item c of this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1053(b)(4):  Ladder(s) were used for purposes other than the purpose for which they were designed:           a) Employees were exposed to the hazard of falling approximately 10 feet while using a stepladder for access and egress between two points on the second story of the building.         In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIV ACTION WORKSHEET).
