29 CFR 1926.652(g)(1)(iv): Employees were allowed in shields when shields were being installed, removed, or moved vertically: (a) Denver Transit Constructors LLC at W 64th Ave & Pecos St., Denver, CO 80221: On July 22, 2015 the exposing employer allowed employees to work inside the excavation shield system when it was being moved vertically, in that employees were working inside the shield system while a Caterpillar 336E excavator was used to lift one of the sides of the shield system. The excavator bucket unexpectedly detached from the boom and fell to the bottom of the trench, thus striking an employee that was working inside the shield system. This condition exposed employees to struck-by hazards.
