29 CFR 1910.22(d)(1): On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate.     On or about July 21, 2015, and times prior thereto, overhead storage area above sewing room was not marked with approved loads.  Employees were exposed to the hazard of being struck by falling equipment and material.

Three (3) doors were held shut by strap and wire or sling. Two doors on the south side of the building and one door on the west side of the building.  Employees were exposed to the hazards associated with fire.

Black electrical tape was wrapped around the cord.  Employees were exposed to the hazard of electric shock.
