29 CFR 1928.57(a)(6): At the time of initial assignment and at least annually thereafter, the employer did not instruct every employee in the safe operation and servicing of all covered equipment with which they were or would be involved: On or about 07/21/15, the employer did not annually instruct the employees performing work and maintenance (to include unclogging) on the machines such as but not limited to the flakers in the mill. Employees were exposed to struck-by and or caught-between hazards.

29 CFR 1928.57(c)(2)(i): The mesh or nip-points of all power driven gears, belts, chains, sheaves, pulleys, sprockets, and idlers were not guarded: Two sets of pulleys on the west flaker located in the mill were not fully enclosed. Employees were exposed to the nip-points of the meshing belts and pulleys.
