Rosa Mexicano at 61 Columbus Ave, throughout the facility materials and equipment such as, but not limited to, bicycles, building materials, and chairs were placed either permanently or temporarily within the exit route, employees were exposed to hazards of smoke inhalation and delayed exit in case of emergency in the following locations: a. First floor kitchen preparation area. b. First level emergency exit to 62nd St. c. Mezzanine level Stair B. The condition was noted on or about July 27, 2015. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 AS PER 29 CFR 1903.19
