29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: a) Second Floor Stairway - On or about 04/29/2015 employees were exposed to a 10 foot fall through an unguarded 15 foot by 4 foot stair opening. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): Where portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: a) Fourth Floor - On or about 04/29/2015, two carpenters were exposed to a possible 10 foot fall from an aluminum extension ladder which projected 2.5 feet above the ladder landing and was not secured in place and provided with a grabrail.
