Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to explosion hazards when cutting into an unpurged compressed gas cylinder: In the scrap area; The employee was operating a shear attachment on a CAT Excavator to cut into an unpurged accumulator with a charged gas bladder containing nitrogen at approximately 2175 psi, exposing the employees to struck by hazards from gas cylinder explosion. ONE FEASIBLE AND ACCEPTABLE MEANS OF ABATEMENT WOULD BE TO: Follow the Handbook of Compressed Gases of the Compressed Gas Association, Inc. CGA P-9 1992, The Inert Gases: Argon, Nitrogen and Helium Chapter 7.

29 CFR 1910.110(f)(2)(i): Containers in storage were not located so as to minimize exposure to excessive temperature rise, physical damage, or tampering from unauthorized persons: Perlman Recycling, 54 South Merriam Street, Pittsfield, Massachusetts; On or about July 27, 2015 liquid petroleum gas tanks that had been purchased as scrap were not stored in a suitable enclosure or protected against tampering. Tanks with and without valves were observed in scrap piles, loose in the "yard", and stored in a cardboard - Gaylord.
