29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. Location: 24 La Grange Street West Roxbury, MA Employee was exposed to a fall hazard of approximately 16 feet, while working from a pump jack scaffold that had no guardrail systems, end rail systems, nor were personal fall arrest systems used or provided. The Garcia Siding and Roofing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451.(g)(1), which was contained in OSHA inspection number 1019143, citation number 1, item number 1 and was affirmed as a final order on 2/27/2015, with respect to a workplace located at 46 Ledgelawn Avenue, Lexington, MA.
