29 CFR 1926.651(c)(2):  A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet or more in depth so as to require no more than 25 feet of lateral travel for employees:     (a)   Integrity Underground, Inc. at W Bromley Ln & Fulton Ave, Brighton, CO 80601:  On or before July 24, 2015 the exposing employer did not ensure that a stairway, ladder, ramp or other safe means of egress was provided to employees that worked in a trench that was approximately 6 feet 2 inches in depth, 3 feet in width, and 7 feet in length.  This condition did not provide employees with a safe means of trench egress.

29 CFR 1926.651(j)(2):  Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into the excavation:     (a)  Integrity Underground, Inc. at W Bromley Ln & Fulton Ave, Brighton, CO 80601:  On or before July 24, 2015 the exposing employer did not ensure that the spoil pile was at least two feet from the edge of the trench.  Employees worked in a trench that was approximately 6 feet 2 inches in depth, 3 feet in width, and 7 feet in length.  This condition exposed the employees to the hazard of spoil pile material falling into the excavation or causing the excavation wall to collapse.

29 CFR 1926.652(a)(1):  Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c):     (a)   Integrity Underground, Inc. at W Bromley Ln & Fulton Ave, Brighton, CO 80601:  On or before July 24, 2015 the exposing employer did not ensure that employees working inside a trench with near vertical walls were protected from cave-in, in that employees used a shovel to locate utility lines without using a protective system.  The trench was approximately 6 feet 2 inches in depth, 3 feet in width, and 7 feet in length.  This condition exposed employees to a crushing hazard by excavation wall cave-in.

29 CFR 1926.651(k)(1):  Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems, hazardous atmospheres and or other hazardous conditions:     (a)  Integrity Underground, Inc. at W Bromley Ln & Fulton Ave, Brighton, CO 80601:  On or before July 24, 2015 the exposing employer did not ensure that daily inspections of a trench, that was approximately 6 feet 2 inches in depth, 3 feet in width, and 7 feet in length, and the adjacent area were made by the competent person for evidence of a situation that could have resulted in a hazardous condition, in that the competent person did not identify that the trench had near vertical walls, undercut sections near the bottom, and contained type B and C soil.  This condition exposed employees to a crushing hazard by trench wall cave-in.     Abatement Note:  "Competent person" means one who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has authorization to take prompt corrective measures to eliminate them.
