29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:         (a)  Monroeville Industrial Moldings, Inc., 75 Ontario Street, Norwalk, Ohio:  On or about July 23, 2015, the employer did not ensure that employees were protected from falling approximately 72 inches to a concrete floor. Employees stood on press #2 in order to clear material from the feed throat and were at risk of serious fall-related injuries.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:         (a)  Monroeville Industrial Moldings, Inc., 75 Ontario Street, Norwalk, Ohio:  On or about July 23, 2015, the lockout procedure for injection press #2 was not utilized when an employee used their hands to remove material from the feed throat of the press.  While removing the material from the throat, a lock was not affixed to the electrical disconnect.  The employee's hands were within 3 inches of the feed auger and at risk of serious injury.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:         (a)  Monroeville Industrial Moldings, Inc., 75 Ontario Street, Norwalk, Ohio:  On or about July 23, 2015, the employer did not ensure that a lockout device was affixed to the electrical disconnect for injection press #2 when an employee used their hands to remove material from the feed throat of the press.  While removing the material from the throat, the electrical disconnect was turned off.  The employee did not affix a lock to the disconnect.  The employee's hands were within 3 inches of the feed auger and at risk of serious injury from the crush and amputation hazard.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:         (a)  Monroeville Industrial Moldings, Inc., 75 Ontario Street, Norwalk, Ohio:  On or about July 23, 2015, the employer did not ensure that employees were adequately trained in the purpose and function of the energy control procedure for press #2.  Employees routinely unjammed the press without affixing a lock to the electrical disconnect.  Employees were unaware of the need to lockout the press.
