29 CFR 1910.132(a):     Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    Employees in the "Outback Snacks" food kiosk known as "Funnel Cakes" are exposed to burn injuries from deep fat fryers and hot splashing oil while cooking food without wearing protective clothing.     Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.138(a):   The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes:  Employees in the "Outback Snacks" food kiosk known as "Funnel Cakes" are exposed to burn injuries from deep fat fryers and hot splashing oil while cooking food without wearing appropriate hand protection.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1904.7(b)(3):   The employer did not correctly classify the following work-related injuries or illnesses on the OSHA Form 300 or equivalent:   The employer did not enter correctly the following work-related injuries on the OSHA Form 300 or equivalent for the calendar year 2015:   a) On or about July 17th, 2015, an injury was recorded on the OSHA Form 300 as having occurred in the funnel cake area involving a fainting episode and heat exhaustion experienced by a food services employee. The employer did not enter a check mark in column H, as required to classify the case as requiring days away from work. Further notation in column K should have read "1" for the one day of work prescribed to be missed by the physician the employee saw following the injury.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
