29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c):    a)  On or about July 28, 2015 and at times prior, employees were not protected from cave-in hazards while installing water main pipes in an unprotected trench.  The trench was approximately 5 feet wide, had vertical walls ranging from 7 to 10 feet in depth in several areas where employees were working.      Gordon Construction Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1) which was contained in OSHA inspection number 452296, citation number 2, item number 1 and was affirmed as a final order on 08/06/2012, with respect to a workplace located at Broadview Terraces, Colorado Springs, CO 80906.
