29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  a.  On the 4th floor beside the electric panel, there was an open floor hole where the laundry chute would be located which was open and not covered over with a cover thereby exposing employees to a 10 foot fall hazard.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers.  a.  On the first floor walkway where employees travel through the building, there were diamond cut holes around 13 columns which measured 24 inches by 24 inches square and 8 inches deep thereby exposing employees to a trip/fall hazard.

29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted.  a.  On the lower roof where an employee was observed laying roof panels, the General Contractor did not ensure the roofing contractor was provided and wore a fall protection system thereby exposing employees to a 10 foot fall hazard.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows:  29 CFR 1926.502(d)(15)(i): as part of a complete personal fall arrest system which maintains a safety factor of at least two;   a.  On the roof of the hotel where employees were installing panels as part of a roofing system the employer did not provide a qualified person to supervise the installation of the fall protection system thereby exposing employees to a fall hazard.

29 CFR 1926.503(a)(2): The employer shall assure that each employee has been trained, as necessary, by a competent person qualified in the following areas:  29 CFR 1926.503(a)(2)(ii): The correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems to be used;  a.  On the site where work was performed on the upper steep pitch roof and the low slope roof employees were installing metal panels for the roof, the employees were not trained in the requirements for fall protection, thereby exposing employees to a 40 foot fall hazard.
