29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous. a. On the side of the building where employees were using a Ridgid Vacuum to pull string through the conduit to pull electric wiring, the vacuum did not have a ground pin thereby exposing employees to an electrical hazard.

29 CFR 1926.405(b)(1): Conductors entering boxes, cabinets, or fittings. Conductors entering boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed. Unused openings in cabinets, boxes, and fittings shall also be effectively closed. a. In the first floor electrical room where the electrical panels which supplied power to the building were located, there were twelve missing circuit breakers thereby exposing employees to an electrical hazard. b. On the second floor of the building, the energized electrical panel used to supply power to the building was missing twelve circuit breakers thereby exposing employees to a electrical hazard.
