29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not protected from the rays by noncombustible or flameproof screens or shields:  a) Fabrication Shop - The employees were not protected with welding screens while workers were performing tact welding.  Employees working in adjacent areas in the fabrication shop were exposed to ultraviolent radiation from the welding operations.

29 CFR 1910.305(b)(2): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:  1) Fabrication Shop - The 208 volt electrical junction box was not completely enclosed with a cover designed by the manufacturer.  The junction outlet box was missing a knockout. 29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  1) Paint Room - The employer failed to provide medical evaluation for all employees who were required to use a North half-mask Air Purifying Respirator model number 7H0N99 during periods where an exposure may exist to determine the employee's ability to use a respirator. Employees were exposed to organic solvents such as Acetylene, Argon and Titanium Dioxide.   2) Fabrication Shop - The employer failed to provide medical evaluation for all employees who were required to use a N-95 Respirator during periods where an exposure may exist to determine the employee's ability to use a respirator.  Employees were exposed to organic solvents such as Acetylene and Argon.

Employees were using hazardous chemicals such as but limited to Argon, Acetylene and Titanium Dioxide.
