29 CFR 1910.1001(j)(2): The Building and facility owners did not determine the presence, location and quantity of ACM and PACM. The employer and building and faculty owners did not exercise due diligence in complying with requirements to inform employers and employees about presence and location of ACM and PACM:    a)  Church of St Aloysius and School, 935 Bennetts Mill Road, Jackson, New Jersey, old church building - A survey to identify and quantify asbestos and/or ACM (asbestos containing materials) in the building was not conducted. Samples of floor tiles and mastic taken on June 26, 2015 during the removal of the tiles revealed that they contained from 1.1% to 4.4% chrysotile. The rest of the building had not yet been surveyed. On or about 7/29/15.    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
