29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):      Location: Harvard Terrace    Instance 1: Employees were expose to fall hazards while working on windows that were not guarded or provided fall protection.    Instance 2: Employees were exposed to fall hazards where door openings were not guarded.    Instance 3: Employees were exposed to fall hazards where a stairwell shaft was left unguarded.    Instance 4: Employees were exposed to falling through floor openings and tripping hazards from openings in the floors.    The Brookline Development was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number  977760, citation number 1, item number 3, and was affirmed as a final order on 7/1/2015, with respect to a workplace located at 50 Dedham Avenue Needham MA.
