29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): a. 61-65 NJ State Route 31, Hampton, NJ Employees engaged in commercial siding installation accessed a Werner Pump Jack scaffold work platform from a welded frame scaffold that was not fully decked. Employees accessed the welded tubular frame scaffold platform using a ladder to the fourth tier which was not fully decked. Employees stood on double width wood planks as a means of a platform for access and/or egress to the pump jack scaffold. Violation observed on or about July 29, 2015

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. a. 61-65 NJ State Route 31, Hampton, NJ Employees engaged in commercial siding installation accessed a Werner Pump Jack scaffold platform which was more than two feet above the access level without the use of a ladder. Employees accessed a welded tubular frame scaffold using a ladder to the fourth tier and climbed from one platform deck to the next using the end rail and then onto the pump jack stage plank used as a bridge for access to the pump jack scaffold. Violation observed on or about July 29, 2015
