OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and struck-by hazards associated with improper automobile lift operations:    (a) On or about July 29, 2015, and at times prior, Cosey, LLC., employees were exposed to crushing and struck-by hazards while working on automotive lifts as follows:    1 - Lifts were not inspected annually by a qualified lift inspector.   2 - Lifts were not inspected daily during use to ensure that they were properly maintained and functional.  3 - Lift #1 Did not have capacity labels, operator did not know its capacity.  4 - Lift #1 Was missing the padding on the swing-arm base.  5 - Lift #2 Worn out padding on the swing-arm bases.  6 - Lift #6 Locks on swing-arms do not engage. 29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  (a) On or about July 29, 2015, and at times prior, Cosey, LLC., did not ensure that the work rests on grinding machinery were adjusted closely to the wheel with a maximum opening of 1/8". Employees were exposed to struck-by hazards, in that an 8" stand-alone grinder was used without the work rest. 29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:    (a) On or about July 29, 2015,and at times prior, Cosey, LLC., did not ensure that flexible cords and cables were connected to devices and fittings, so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminals. A 4-way, 110V receptacle mounted on the side of lift #2, showed signs of strain, inner wiring was visible, exposing employees to contact with electrical hazards. Employees were exposed to contact with electrical hazards, in that 110-V receptacles in the service shop, and in the detailing area were broken, specific locations were:  1- West wall of station #2 2- East wall of station #4 3- West wall of detailing area. 4- Detailing area partition.
